POEM is an endoscopic myotomy that creates a submucosal tunnel in the oesophagus to cut selected muscle fibres for achalasia or related motility disorders after manometry has confirmed the diagnosis.
Under anaesthesia, a mucosal incision is made, a submucosal tunnel is created and a planned length of circular muscle is cut. The incision is then closed with clips.
POEM is not a fundoplication and does not treat every cause of dysphagia. Reflux can occur after myotomy. G-POEM and Z-POEM are different CMS procedures.
